Independent Activity Form
Complete this form to obtain Others First credit after you have completed an Independent Activity. (If you have trouble accessing Adobe PDF documents, download the Adobe reader) This form requires a signature from a representative of the independent community service agency, validating the number of service hours. Because independent activities fall outside the jurisdiction of the program they do not require District permission slips.
